The following forms are not available using My State Bar Profile. They must be printed and submitted by mail or FAX:

  • Membership Records Name Change
  • Application for Change of Membership Status to Inactive
  • Application for Change of Membership Status to Active 
  • Transfer of Estate Planning Documents
